Heart failure (HF) is a chronic progressive disease with over 6.5 million Americans affected, and researchers expect the number to increase to 8 million by the year 2030. Nearly all HF patients will have at least one hospitalization during their disease and one in four patients face readmission within 30 days of discharge.  In this complicated disease there is nearly a 40% mortality rate within a year of their first hospitalization, taking a tremendous toll on the quality of life for patients, families, and caregivers.  Tune in on December 1st to hear from national thought leaders on HF prevention and management and to network with like-minded colleagues invested in improving outcomes for patients with a HF diagnosis.
